Medical Glass Manufacturer Cited for Failure to Train Workers in LOTO

During a random inspection in 2023, OSHA concluded that Medical Glass did not properly train its
employees in lockout/tagout procedures. OSHA returned for a follow-up inspection in 2024 and
found that the unsafe conditions continued. Employees were not adequately trained to prevent
exposure to operating machinery during service work.

OSHA cited Medical Glass for:

  • Failure to train workers in required LOTO procedures
  • Ongoing exposure to hazardous machine motion
  • Repeat violations for previously identified hazards
