During a random inspection in 2023, OSHA concluded that Medical Glass did not properly train its
employees in lockout/tagout procedures. OSHA returned for a follow-up inspection in 2024 and
found that the unsafe conditions continued. Employees were not adequately trained to prevent
exposure to operating machinery during service work.
OSHA cited Medical Glass for:
- Failure to train workers in required LOTO procedures
- Ongoing exposure to hazardous machine motion
- Repeat violations for previously identified hazards
Medical Glass received multiple repeat violations and fines.
