A tube handler was positioning a 6,000-pound bundle of steel tubes when his arm was caught
between a rotating roller and a steel tube bundle resulting in severe arm and wrist injuries. OSHA
discovered that the employer had not corrected previously documented hazards and failed to train
workers in essential safety procedures.
OSHA cited ArcelorMittal for:
- Failure to train workers on procedures to reduce exposure to operating machine parts
- Repeat violations of machine guarding requirements
- Failure to ensure proper lockout/tagout procedures
ArcelorMittal received penalties of $253,515
